Oncology Patient Navigator, RN

Requirements:

Education: Bachelor’s or Master’s degree in nursing from an accredited school of nursing preferred.

Experience: Minimum 2 years Oncology experience preferred.  Experience in multiple hospital departments preferred.

Licenses/Certification: RN License current in the state of Colorado or compact state. Current BLS certification.  ONC Certification at time of hire or within 1 year of hire.

Responsibilities:

As the Oncology Nurse Navigator at Gunnison Valley Health you’ll be responsible to provide individualized assistance to patients, families and caregivers; to guide the patient and the family through the health system from diagnosis, testing, treatment and follow up care. You will utilize your unique skills and abilities by:

  • Facilitating care within the context of functional status, cultural consideration, health literacy, psychosocial and spiritual needs for patients.
  • Developing and maintaining an effective liaison role between patient and other medical team members.
  • Educating the patient, family, and caregivers regarding treatment options, community resources, and self-care management throughout all phases of the process.
  • Partnering with the GVH Marketing team to conduct ongoing promotion of the healthcare program.

Physical Requirements:

Occasionally - (1-33%) Walking, sitting, crouching/squatting, stooping, stairs, lifting (0-20 lbs), carrying (0-20 lbs), pushing/pulling (20-50 lbs)

Frequently - (34 - 66%) Standing, walking, change position, reaching, reach across midline, handling, pinching

Continuously - (67 -100%) Must be able to see with corrective eyewear, must be able to hear clearly with assistance

Benefits Eligibility:

Medical, dental, vision, healthcare FSA, dependent care FSA, and Lifestyle Spending Account:  All active employees working 40 or more hours per pay period in a Full Time or a Part Time position are eligible for benefits on the first of the month after hire.

Life and AD&D insurance: All active employees working 40 or more hours per pay period are eligible for benefits on the first of the month after hire date.

Short term and long term disability: All active employees working 60 or more hours per pay period are eligible for benefits on the first of the month after hire date.

Compensation: $36.51 - $47.00 /hour DOE
