Job Description – Neighbors Housing Neighbors Case Manager – Cumberland County – Reformed Church of Highland Park Affordable Housing Corporation (RCHP-AHC)
Position Description:
The Neighbors Housing Neighbors Case Manager provides comprehensive, culturally responsive case management services to eligible participants enrolled in the Neighbors Housing Neighbors program. This role delivers housing-focused case management and referral services to tenants residing in units subleased by RCHP-AHC throughout Cumberland County. The Case Manager works to support housing stability, connect tenants to resources, and promote successful tenancies.
Responsibilities:
Working with the CEO or Tenant Services Liaison to meet with potential clients for Lease/Sublease Housing who don’t qualify for RCHP-AHC’s other housing or social service programs.
- Assisting in determining funding for housing (tenant income/voucher eligibility etc.)
Serving as liaison between housing agency, tenants, and supportive services providers Responding to tenant crises and providing information, direct assistance, referrals, and encouragement to tenants as needed Making routine visits to select properties to collect rent and check for maintenance concerns.
Working with tenants who have lease violations to insure they have access to support needed to help them retain their housing. Interfacing with public officials and staff of other non-profits
- Assist in preparing reports for state, county, and municipal funders
Participating in Middlesex counties continuum of care committees (COC) Complete client intakes and screenings to understand needs Collect required documents to determine eligibility (ID, income, benefits, court notices, utility bills, etc.)
- Follow up with clients to ensure all documents are received
Check eligibility for other assistance programs first, including the Homelessness Prevention Program (HRP) and Board of Social Services (BOSS)
- Assist clients with applying for these programs when eligible
Obtain proof of denial before approving Homeless Diversion assistance
- Provide referrals and resources if a client is not eligible
Enter and maintain accurate client records
- Follow up with landlords, courts, utilities, and partner agencies as needed
- Maintain organized and audit-ready client files
Other tasks as assigned
Qualifications:
Minimum bachelor’s degree in social work or related field. Excellent written and oral communication skills.
Experience working with diverse populations:
Strong organization and follow-up skills Ability to manage multiple cases Clear communication skills Bilingual skills preferred
COMPENSATION: This position is 40 hours/week and is $62,500/year plus benefits
