At Corrections Health, we provide medically needed health care to adults and youth in custody at Multnomah County Jails and Juvenile Detention Facility. Come join our team of Portland's most outstanding health care professionals providing exceptional services to our diverse community! The role of On-Call Transition Services Program (TSP) Advanced Practice Clinician (On-Call TSP APC) is to provide comprehensive nursing care and treatment for justice involved individuals within Multnomah County.

    The On-Call TSP APC will be under the clinical supervision of the Corrections Health Medical Director and will function as the main medical provider for the Transition team; guiding clinical care with a focus on medication supported recovery and harm reduction, and collaboration with Health Department and community providers.

    As a On-Call TSP APC Health, you will help serve in The Multnomah County Inverness Jail and Multnomah County Detention Center. As the On-Call TSP APC , other duties will include: Clinical Care Diagnose and treat acute and chronic medical conditions according to scope of practice. This would include: Conducting a medical history, Conducting physical exams Ordering and interpreting diagnostic tests Devising a course of treatment including Medication Supported Recovery (MSR) Discussing alternative treatment; the risk and

    • Documenting appropriately in EHR system Consultation Provide consultation to nursing and other Corrections Health staff as indicated for continuity of client care.
    • Provide in-service to staff as a group or individually, as requested and precept students when assigned.
    • Assist in the development and implementation of protocols, procedures and other teaching tools.

    Attend provider meetings as scheduled and morning shift report.

    • Contribute to patient care planning and identification of patient needs.
