Description:
The Admissions Coordinator plays a critical role in ensuring appropriate patient placement within inpatient and EmPATH (Emergency Psychiatric Assessment, Treatment, and Healing) services. This individual is responsible for vetting all potential admissions, ensuring patients meet medical and clinical criteria, and facilitating smooth transitions into care.
Key Responsibilities:
Clinical Review & Admission Coordination Review patient charts and documentation to assess eligibility for inpatient and EmPATh admission.
- Ensure patients meet established medical and behavioral health criteria.
- Collaborate with attending physicians and clinical teams to confirm appropriateness of admission.
- Document and track admission decisions and rationale.
Referral Management Receive and manage referrals from internal departments, external providers, and community partners.
- Maintain accurate records of referral sources and outcomes.
- Build and sustain relationships with referral partners to support ongoing patient flow.
Communication & Collaboration Serve as the primary liaison between referring providers, clinical teams, and admissions staff.
- Communicate clearly and professionally with physicians, nursing staff, and case managers.
- Provide timely updates to stakeholders regarding admission status and patient placement.
Compliance & Documentation Ensure all admissions comply with regulatory, organizational, and payer
requirements:
- Maintain thorough documentation of patient eligibility, referral details, and admission decisions.
- Assist with audits and reporting as needed.
- Process Improvement & Reporting Identify trends and barriers in the admissions process and recommend improvements.
- Participate in quality improvement initiatives related to patient access and throughput.
Generate reports on referral volume, admission rates, and turnaround times. Patient Advocacy & Support Ensure patients are placed in the most appropriate level of care. Advocate for timely access to services and continuity of care.

Bachelor’s degree in Nursing Clinical
